Patient rights and responsibilities
You have the right to
  • You have the right to respect for your culture, psychosocial, spiritual, personal values, belief, and preferences.
  • Know the name of the physician who has primary responsibility for coordinating your care.
  • Make decisions regarding medical care, and receive as much information about any proposed treatment or procedure as you may need in order to give informed consent or to refuse a course of treatment.
  • Request or refuse treatment, you have the right to leave the hospital even against the advice of physicians. However, you do not have the right to demand inappropriate or medically unnecessary treatment or services.
  • Have personal privacy respected.
  • Confidential treatment of all communications and records pertaining to your care and stay in the hospital.
  • Be informed by the physician, or a delegate of the physician, of continuing health care requirements and options following discharge from the hospital.
  • File a grievance: If you want to file a grievance, you may do so by writing to: Department of Social Service, Shuang Ho Hospital(Add: No.291, Zhongzheng Rd., Zhonghe District, New Taipei City, 23561, Taiwan,(R.O.C.))

 

Patient Responsibilities

  • I will supply accurate and complete information about present complaints, past illness, hospitalizations, medications, and other matters relating to my health.
  • I will make it known when I do not clearly comprehend a course of action or what is expected of me, including, but not limited to informed consent.
  • I will assure that the financial obligations of my health care are fulfilled as promptly as possible.
  • Any complaints about your treatment will be resolved in Taiwan where you are treated,According to itslaws and procedures.
  • All disputes relating to your medical care at Shuang Ho Hospital shall be finally settled by arbitration referred to the arbitration association of the republic of China.The place of arbitration shall be in Taipei, Taiwan. The award rendered by the arbitrator(s) shall be final and binding upon both parties concerned.
